Positive Behaviour Support (PBS) Plans

NDIS behaviour support services offered by Behaviour Collaborations team conducting a Positive Behaviour Support planning meeting with a practitioner, a teen and their parents

At Behaviour Collaborations, we understand how crucial it is for families and NDIS Support Coordinators to feel confident that participants are receiving the right kind of behavioural support - support that is not only effective, but respectful, ethical and tailored to the individual. All providers and workers are expected to uphold the NDIS Code of Conduct. That is why our Positive Behaviour Support plans (PBS plans) are developed collaboratively, with active involvement from the participant, their family, carers, support staff and their broader care team. Our PBS plans are designed to be practical behaviour support plans that can be used consistently across home, school, and community settings.

Our approach is person-centred and partnership-driven. We value and rely on the insights and experiences of both families and NDIS Coordinators who know the participant's journey, challenges and strengths. Together, we work toward shared goals that focus on improving quality of life and long-term well-being.

As part of this work, we review existing reports, observe participants in their everyday environments, and gather information from families, schools, and support workers so that plans are grounded in each person's real-life needs.

We operate from a strengths-based framework, meaning we focus on identifying what is working well and building upon the participant's existing abilities, preferences, and personal aspirations. Rather than focusing only on behaviours of concern, we invest in skill development, communication strategies, emotional regulation and independence, supporting the participant to thrive across all areas of life.

support-coordinators-service

An important part of our role is to implement practical, evidence-based strategies that reduce or remove the need for restrictive practices, ensuring that all interventions uphold the participant's rights, safety, and dignity. Our plans are designed to be flexible, clear, and easy to implement for both families and frontline support workers, so they feel equipped and confident to support the participant consistently and compassionately.

For NDIS Support Coordinators, we provide clear documentation, transparent communication and ongoing collaboration to ensure our Positive Behaviour Support plans align with the participant's NDIS goals and reporting requirements. For families, we offer a supportive and inclusive process where your voice is valued, your concerns are heard, and your hopes for your loved one are central to every plan we create.

We aim to be transparent with our practices so that participants, families, carers, support coordinators, and other providers have a detailed understanding of how we go about our work.

How our behaviour support plans work under the NDIS

Behaviour Collaborations follows a structured six stage process so that all Positive Behaviour Support plans are thorough, collaborative and aligned with NDIS requirements. This process helps us understand the person, identify what is happening around behaviours of concern, and design practical strategies that support safety, skills and quality of life.

  • stage-review

    Stage 1 - Review

We review existing reports and documents, speak with the Support Coordinator or referring professional, and gather any missing information, such as allied health reports. Our team coordinates initial home or external visits in settings such as hospitals, prisons, schools, SDA houses, family homes or community locations. We also meet with key stakeholders and maintain email communication so everyone is informed and working together from the start.

  • Stage-Observe

    Stage 2 - Observe

We prepare for visits with targeted assessments and keep detailed case notes in line with NDIS registration and accountability requirements. A behaviour record link and QR code are created so behaviours of concern can be recorded consistently and in real time. We observe the participant across different environments and support parents and caregivers through coaching, goal setting, and safety planning.

  • Stage-Research

    Stage 3 - Research

Our team completes diagnosis specific research to understand traits, common behaviours and barriers that may affect daily life. We review and correlate existing reports and assessments and explore evidence based strategies for behaviour management. Throughout this stage, we continue contact with the participant and their support network to test our understanding and refine the approach.

  • Stage-Report

    Stage 4 - Report

We prepare letters of recommendation that can support NDIS funding, paediatric reviews or assistive technology requests. Comprehensive Positive Behaviour Support Plans rae developed, with goals that focus on building skills, strengthening relationships and reducing behaviours of concern. Where needed, we complete a Functional Behaviour Assessment to understand the function of behaviours and support further funding applications. We also identify any restrictive practices and submit interim reports to the NDIS Commission when required, and develop participant led safety plans to reduce risk for everyone involved.

  • Stage-Train

    Stage 5 - Train

Using the completed behaviour support plan and supporting documents, we create tailored training materials for families, support workers and other stakeholders. Training is usually delivered in a dedicated session, online or in person, to explain the plan and demonstrate how to implement strategies in daily routines. Travel is included where training is provided face to face.

  • Stage-Recap

    Stage 6 - Recap

As part of ongoing support, we provide follow up or refresher training for existing and new staff, offer Q&A on strategy implementation, and hold periodic stakeholder meetings. A progress report is prepared ahead of plan review to outline what has changed, what barriers remain and any additional supports or funding that may be needed for the next NDIS plan period.

FAQs: Positive Behaviour Support Plans

How long does it take to develop a Positive Behaviour Support Plan?

The timeframe depends on the participant’s needs, level of risk, support environment and the information required.

Where regulated restrictive practices are involved, an interim behaviour support plan must be developed within 1 month of engagement by the specialist behaviour support provider. A comprehensive behaviour support plan must be developed within 6 months of engagement.

Behaviour Collaborations works with participants, families, carers, providers and support teams to gather the right information, understand what is happening and develop practical behaviour support strategies without unnecessary delay.

Can a Positive Behaviour Support Plan be used across different settings?

Yes. A strong Positive Behaviour Support Plan should be practical across the places where support happens, including home, school, community, supported living, work or other daily environments.

Behaviour Collaborations works with families, support workers, schools, providers and other stakeholders so the plan can be understood and applied consistently. A plan that only works in one setting is unlikely to support the participant well in real life.

Who funds Positive Behaviour Support Plans under the NDIS?

Positive Behaviour Support Plans may be funded through a participant’s NDIS plan when behaviour support funding is included and the support is reasonable and necessary for the participant’s needs and goals.

If you are unsure whether the participant’s current plan includes behaviour support funding, Behaviour Collaborations can talk with you about the next steps and help you understand what information may be needed.

At Behaviour Collaborations, we believe Positive Behaviour Support is about more than reducing behaviours of concern. It is about empowering individuals, strengthening support systems, and creating safe, nurturing environments where participants can truly live the life they choose.

Counselling

Mental Health and Wellness Support for All Ages

counselling-mental-health-support

At Behaviour Collaborations, we are committed to supporting the mental health and emotional wellbeing of individuals across all stages of life, from children and adolescents to adults and older people. Our team of qualified and compassionate professionals are experienced in delivering personalised counselling and therapeutic support tailored to the unique needs of each person and their family. In many cases, this work is funded as NDIS counselling and mental health support within a participant’s plan, or accessed through private funding or DVA pathways where applicable.

We provide evidence based support for a wide range of mental health challenges, including but not limited to anxiety, depression, post traumatic stress disorder (PTSD), trauma related concerns, substance use and addiction, schizophrenia, major mood disorders, personality disorders, acquired brain injury (ABI) and fetal alcohol spectrum disorder (FASD).

Understanding that mental health experiences can differ greatly across age groups, we take a developmental and person centred approach, ensuring that our services are age appropriate, culturally sensitive and relevant to each individual's stage of life. Whether it is supporting a young person through early signs of emotional distress or helping an adult manage long standing mental health conditions, we aim to create a safe, respectful and non-judgemental environment for healing and growth.

We also recognise the importance of the right therapeutic match. That is why we take the time to connect individuals with specialists who not only hold the appropriate qualifications but also align with their communication style, preferences, and personal goals. In many cases, this work is funded as NDIS counselling and mental health support within a participant's plan, or accessed through private funding or DVA pathways where applicable.

Therapeutic supports are provided to assist participants from 7 years of age to apply their functional skills to improve participation and independence in daily, practical activities in areas such as communication, personal care, movement, and community living.

FAQs: Counselling

Can I access counselling at Behaviour Collaborations without an NDIS plan?

Yes. Counselling at Behaviour Collaborations may be available to privately funded clients and DVA clients, not only NDIS participants.

Our counselling and behaviour support services are separate but can work alongside each other where appropriate. This allows individuals and families to access support that suits their situation, goals and funding arrangement.

How do I know whether my family member needs counselling or a behaviour support plan?

Counselling and behaviour support can support different needs.

Counselling may focus on emotional wellbeing, mental health, personal challenges and therapeutic goals. A behaviour support plan focuses on understanding behaviours of concern, reducing risk, improving daily support and guiding the people around the participant.

In some situations, both supports may be useful. Behaviour Collaborations can talk through the participant’s needs and help identify which pathway may be most appropriate.

At Behaviour Collaborations, our focus is not just on managing mental health conditions, but on promoting long term wellness, resilience and a sense of belonging in everyday life. We believe that with the right support, everyone has the potential to thrive.

Assessments, reports and letters of support

ndis-reports-assessments-participant-centred

We provide comprehensive support in preparing essential documentation for participants engaging with the National Disability Insurance Scheme (NDIS). Our team is skilled in drafting detailed, accurate and participant centred reports, letters and assessments tailored to meet the specific requirements of the NDIS framework. We prepare NDIS reports and assessments that clearly describe each person's support needs and goals.

For some situations this may include a structured 10 Hour Assessment Report for upcoming plan reviews or changes in circumstances, or a Functional Behaviour Assessment when additional detail is needed to support Positive Behaviour Support funding and recommendations.

  • NDIS-reports

    NDIS reports

Our reports are carefully structured to capture critical information regarding a participant's functional capacity, progress towards goals, and support needs. These documents are designed to clearly communicate evidence-based observations and recommendations, supporting participants' access to appropriate funding and services.

  • NDIS-letters

    NDIS letters

We assist in the creation of formal letters for a range of purposes, including plan reviews, appeals and additional funding requests. These letters are crafted to advocate effectively on behalf of participants, presenting clear and persuasive arguments aligned with NDIS policies and participant goals.

  • NDIS-assessments

    NDIS assessments

Our assessments involve thorough evaluations conducted in collaboration with participants, families, and other stakeholders. We focus on identifying strengths, challenges, and areas requiring support, ensuring that recommendations are practical and participant-focused. These assessments are compliant with NDIS guidelines and contribute to the development of individualised support plans.

FAQs: Assessments, Reports & Letters

What is the difference between a Functional Behaviour Assessment and a standard NDIS report?

A Functional Behaviour Assessment looks closely at behaviours of concern and what may be contributing to them. It considers what happens before, during and after the behaviour, and what the behaviour may be communicating or achieving for the person.

A standard NDIS report may document broader information, such as functional capacity, progress, support needs, risks or recommendations.

Behaviour Collaborations may recommend different reports depending on the participant’s needs, funding, goals and current support situation.

Can Behaviour Collaborations write letters to support an NDIS plan review?

Yes. Behaviour Collaborations can prepare letters of support for NDIS plan reviews, changes in circumstances or funding-related requests where this is appropriate.

These letters are written to explain the participant’s support needs, current concerns and the reason behaviour support may be required. If a plan review is coming up, it is best to contact the team early so there is enough time to review information and prepare suitable documentation.

By leveraging our expertise, participants can feel confident that their documentation will be professional, thorough and designed to maximise their opportunities within the NDIS system.

Transitioning

NDIS transition planning and Behaviour Support Plans

Transitioning

Transitioning through different stages of life or changing care environments can be challenging, especially for individuals with disabilities or complex support needs. At Behaviour Collaborations, we specialise in NDIS transition planning and person-centred behaviour support as participants navigate key life transitions, including moving from hospital to home, reintegrating after incarceration, transitioning from aged care facilities and stepping into adulthood. Our behaviour support plans help keep routines, roles, and safety steps clear during periods of change.

Each of these transitions often involves significant adjustments that can impact a person's behaviour, wellbeing and overall quality of life. Our approach recognises the unique complexities of these moments and focuses on providing tailored, person-centred behaviour support to ensure a smooth and positive experience.

  • Transitioning-from-hospital

    Transitioning from hospital

Leaving hospital and returning to community or home settings can be overwhelming, especially when new routines, medications, or support requirements are introduced. Our behaviour support plans are designed to address any behaviours of concern that may arise due to the stress or changes associated with hospital discharge. We work closely with hospital teams, families, and community support to develop strategies that promote stability, safety, and confidence during this critical time.

  • Reintegration-from-prison

    Reintegration from prison

Re-entering the community after incarceration can involve significant emotional and social challenges. Our specialists collaborate with justice and community services to create behaviour support plans that assist participants in adapting to new environments, managing stressors and reducing behaviours that might otherwise lead to re-offending or social isolation. We prioritise dignity, empowerment and connection in our approach.

  • Transitioning-from-aged-care

    Transitioning from aged care

For older adults moving out of aged care facilities and into more independent living arrangements or alternative supports, behaviour support plans are crucial in managing changes in daily routines, social interactions and health needs. Our team supports individuals and their families in navigating these transitions with compassionate, strengths-based strategies that foster wellbeing and autonomy.

  • Moving-into-adulthood

    Moving into adulthood

The shift from childhood or adolescence into adulthood is a pivotal stage that brings new responsibilities, freedoms, and sometimes challenges. Our behaviour support plans focus on skill building, communication and self advocacy to help young adults navigate education, employment, social relationships and independent living. We partner with families, schools and community organisations to ensure continuity and consistency of support.

  • How-we-support-you

    How we support you

At Behaviour Collaborations, our behaviour support plans are personalised and developed collaboratively with the participant and their support networks. We use evidence based practices to reduce or eliminate behaviours of concern and promote positive behaviours that support independence and community participation. Throughout all transitions, we prioritise your goals, preferences and rights, ensuring that the support provided respects your dignity and enhances your quality of life.

As part of this, we can also develop safety plans to reduce any immediate risk to the participant and the team when behaviours of concern or significant changes are present.

FAQs: Transition Planning

When should transition planning begin for someone leaving hospital, aged care or another care setting?

Where possible, transition planning should begin early, ideally before the move or discharge takes place.

Early planning gives the team time to understand the participant’s routines, risks, communication needs, support requirements and environmental changes. This can make the transition safer and reduce the chance of avoidable stress or escalation.

Behaviour Collaborations can support planning before, during and after a transition where behaviour support needs are present.

Can an existing behaviour support plan be updated for a transition period?

Yes. An existing behaviour support plan can often be reviewed and updated to reflect a new environment, routine or support arrangement.

This may be more efficient than starting from the beginning, especially when the participant’s history, strengths, triggers and existing strategies are already known.

A transition review can help the new support team understand what is changing, what needs to stay consistent and what strategies may need to be adjusted.

Navigating change is easier when you have a trusted partner by your side. Whether you are transitioning from hospital, prison, aged care or moving into adulthood, Behaviour Collaborations is here to provide the expertise, compassion and practical support you need every step of the way.

  • Emotional Regulation: Helping participants identify, understand, and manage their emotions in a supportive environment, fostering self-awareness and resilience.
  • Behavioural Development: Addressing behaviour-related challenges by promoting positive behavioural changes through structured activities that reinforce adaptive skills.
  • Social Skills: Enhancing communication, cooperation, and relationship-building abilities through interactive play.
  • Cognitive and Motor Skills: Supporting developmental progress in areas such as problem-solving, attention, and fine and gross motor coordination.
play-therapy

Delivered by qualified practitioners with expertise in therapeutic play techniques, our play therapy is tailored to each participant's unique needs and goals. This approach aligns with NDIS objectives by empowering participants to gain greater independence, confidence, and well-being.

FAQs: Play Therapy

What age group is play therapy suitable for at Behaviour Collaborations?

Play therapy at Behaviour Collaborations is available for NDIS participants from 7 years of age.

It may be suitable for children and young people who find it difficult to express themselves verbally, are experiencing emotional regulation challenges, or need support with social skills, confidence, coping strategies or behaviour-related concerns.

Suitability depends on the participant’s needs, goals and funding.

Is play therapy covered by an NDIS plan?

Play therapy may be funded through an NDIS plan where the support is reasonable and necessary and aligns with the participant’s goals and funding categories.

Depending on the plan, play therapy may relate to capacity building supports such as Improved Daily Living or Improved Relationships. Behaviour Collaborations can help you understand whether play therapy may be suitable and whether supporting documentation is needed.

Telehealth Services

Telehealth refers to the provision of Positive Behaviour Support (PBS) through telepractice methods, enabling support to be delivered remotely rather than in-person. Telehealth refers to the provision of Positive Behaviour Support (PBS) through telepractice methods, enabling support to be delivered remotely rather than in-person. Telehealth behaviour support can be delivered through our Telehealth services to support access, continuity, and consistent implementation when travel, distance, or other barriers make in-person sessions difficult.

Tele-practice broadly encompasses the delivery of professional services using communication technologies that bridge physical distance between the practitioner and the client. While many professionals commonly rely on face-to-face videoconferencing as their primary mode of tele-practice, this approach can extend beyond video calls to incorporate a variety of other communication modalities.

telepbs-remote-support

These include:

  • Audio-only videoconferencing: Interaction through voice communication without the use of video, which can be useful in situations where video access or bandwidth is limited.
  • Text-based videoconferencing: Using chat boxes or instant messaging features embedded in video platforms to communicate in real time through written messages.
  • Telephone communication: Traditional voice calls that facilitate direct and immediate verbal interaction without internet dependency.
  • Email correspondence: Asynchronous communication allowing detailed information sharing, documentation and follow up through written messages delivered electronically.
  • Hybrid or combined approaches: Integrating two or more of the above methods to tailor the delivery of Positive Behaviour Support based on client preferences, accessibility and specific contextual needs.

FAQs: Telehealth

Is telehealth behaviour support as effective as face-to-face support?

Telehealth behaviour support can be effective for many participants, families and support teams, particularly where travel, distance, health, scheduling or regional access makes face-to-face appointments difficult.

Telehealth works best when families, carers, providers and support workers are actively involved and able to apply strategies between sessions.

Behaviour Collaborations may recommend telehealth, in-person support or a combination of both depending on the participant’s needs, goals, location and support environment.

What do I need to access telehealth behaviour support through Behaviour Collaborations?

You need a device with a camera and microphone, a stable internet connection and a private space where you can talk comfortably.

No specialist setup is usually required. Telehealth can be a practical option for participants and families outside face-to-face service areas, or where online appointments make it easier to maintain consistent behaviour support.

Telehealth aims to maintain the effectiveness and person-centred nature of Positive Behaviour Support while overcoming geographical, logistical, or physical barriers that may limit access to traditional face-to-face services.
